Kodak Brownie Junior Survey and Instructions
This item is a classic example of photographica ephemera. A customer survey card is attached by a green string to a Brownie Junior instruction manual. A second card advertises Kodak 616 film, and on the back, camera accessories for sale. In decades of collecting, this is the only example I have seen. I would guess that most of these cards were either submitted as requested, or torn from the manual and thrown away.

Kodak Brownie Junior Survey and Instructions
The back of the survey card simply asks for the customer's name, address and camera model. I assume that after the card was returned, Eastman Kodak mailed a questionnaire to the camera owner.
